We collaborate every day—during conversations, meetings, decisions, and peer interactions. Yet few teams pause to reflect on how collaboration actually plays out in practice.

This short questionnaire invites you to assess how your team typically works together in

everyday situations. The questions focus on observable team patterns. It’s not an assessment of your individual actions, but your perception of how the team as a whole tends to operate.

Please base your answers on your experience with this team over the past 4–6 weeks, and think “we,” not “me”.
